Request a Business Associate Agreement

If your organization is a HIPAA covered entity or business associate and you plan to use New Century Labs to perform services involving PHI on your behalf, you can request a Business Associate Agreement review here.

Not every healthcare relationship requires a BAA. Some disclosures between healthcare providers for treatment, or between a provider and a laboratory for treatment purposes, may not require one. Send us the details and we will review the workflow.

What to include

  • Legal name of your organization and any DBA.
  • Your organization type, such as provider group, clinic, health plan, business associate, employer, wellness program, or technology vendor.
  • Primary legal, compliance, or operations contact.
  • Description of the services you want to use and the PHI involved.
  • Whether you want to use NCL's BAA template or send your own template for review.
  • Desired launch date or deadline.

  1. We review your organization type and intended workflow.
  2. We confirm whether a BAA appears appropriate for the relationship.
  3. We exchange the BAA template and any required security or compliance information.
  4. Once signed, we keep the agreement on file and coordinate launch details.
